Plastic chair
Seating height 15 3/4 inches H 27 1/2 " by W 24 7/8" by D 24" Neutral shell NL vinyl edge

This arm chair comes from the collection of Annette Del Zoppo, who worked at the Eames Office from 1961 to 1970.

In 1975, she produced this article, The Multi-Dimensional Worlds of Charles and Ray Eames, for the LA Times.

It features the renowned black and white checkered fabric designed exclusively for Herman Miller by Alexander Girard.

This example was exhibited at the New Mexico State University exhibition on Girard's work, called MODERN DESIGN/FOLK ART and a close up of this chair was featured on the catalog and postcard for the show, as shown here.

Here is a a link to a very informative article about Girard's work by Alexandra Lange, who wrote the catalog essay for the NMSU catalog of this exhibition.
